I would like a copy of the SWITCH tool software and I agree to use the software under the following conditions.

1. Intended Use and Audience

SWITCH contains health economic information intended for payers and formulary or population health decision-makers. At the health-plan level, it is designed to estimate projected healthcare costs, relapses avoided, readmissions avoided, and changes in antipsychotic adherence associated with switching eligible populations with schizophrenia from oral antipsychotics to once-monthly paliperidone palmitate (PP1M), including subsequent transitions from PP1M to once-every-three-months paliperidone palmitate (PP3M) and once-every-six-months paliperidone palmitate (PP6M).

The tool is intended to support formulary or population-level discussions about budget impact, cost offsets, and resource utilization. It is not intended for use by medical practitioners making prescribing decisions for individual patients.

The model presents illustrative projections based on its assumptions and inputs. Each user must evaluate costs using the user's own negotiated reimbursement rates, rebates, population characteristics, and other relevant factors.

2. Limitations and Disclaimer

SWITCH is not a diagnostic tool and is not intended to direct treatment for an individual patient. The analyses do not adjust for illness severity or all patient characteristics, including socioeconomic status, ethnicity, and regional influences.

The analyses also may not account for the full clinical circumstances presented at the point of care, including medical history, coexisting conditions, and their management. They are not a substitute for well-controlled comparative clinical trials or clinical judgment.

Model outputs are projections and may differ from actual results because of the assumptions selected, the accuracy and appropriateness of the inputs, population and utilization patterns, reimbursement arrangements, and real-world outcomes. Johnson & Johnson and its affiliates are not responsible for such differences. Neither the model's assumptions nor its outputs imply or guarantee discounts, rebates, cost savings, adherence improvements, relapse reductions, readmission reductions, or other clinical or economic outcomes.

Product or cost comparisons are not statistically tested and are not intended to imply similar or superior efficacy of any product. Refer to the full prescribing information for each medication before proceeding.

3. Demonstration and Distribution Restrictions

I may demonstrate or present SWITCH within my organization solely for the purposes described in Section 1. I may not copy, provide, transfer, sublicense, or otherwise distribute the Excel model to any third party or to any person outside my organization.

4. Sharing of Results

I may share analyses, results, or outputs generated using SWITCH only for purposes consistent with Section 1 and only with recipients authorized to receive them. Any shared output must be presented accurately, identify the relevant assumptions and data sources, provide sufficient context for interpretation, and not disclose or enable reconstruction of the model or other proprietary content. I acknowledge that SWITCH does not make claims regarding the indicated uses, comparative efficacy, or safety of the products evaluated.

5. Ownership and Limited License

SWITCH is licensed, not sold. Johnson & Johnson, its affiliates, or their licensors retain all right, title, and interest in and to SWITCH, including all associated intellectual property and proprietary rights. Subject to my compliance with these terms and conditions, Johnson & Johnson grants me a limited, non-exclusive, non-transferable, non-sublicensable, and revocable license to use SWITCH solely for the purposes described in Section 1. No rights are granted by implication, estoppel, or otherwise, and all rights not expressly granted are reserved by Johnson & Johnson, its affiliates, and their licensors.

6. Data Privacy and Use Restrictions

I will not enter, upload to, or otherwise use with SWITCH any information that directly identifies an individual or could reasonably be used to identify an individual, including protected health information, personal data, or other sensitive or confidential information. I will use only aggregated, anonymized, or properly de-identified data that I am legally authorized to use for the purposes described in Section 1. I am responsible for ensuring that all data used with SWITCH are lawfully obtained, accurate, appropriately secured, and handled in compliance with applicable laws, regulations, contractual obligations, and organizational policies. I will not attempt to re-identify any individual or combine SWITCH data or outputs with other information for that purpose. Johnson & Johnson and its affiliates are not responsible for data entered into or used with SWITCH by me or on my behalf.

7. Termination and Deletion

Johnson & Johnson may suspend or terminate my right to use SWITCH at any time, with or without notice, including if I breach these terms and conditions. Upon termination, expiration, or written request from Johnson & Johnson, I will immediately cease all use of SWITCH and permanently delete or destroy every copy in my possession, custody, or control. I will not retain, access, or use SWITCH thereafter. Upon request, I will provide written certification that I have complied with these obligations. Termination does not limit any other rights or remedies available to Johnson & Johnson. Sections 2, 4, 5, 6, and 7, together with any other provisions that by their nature are intended to survive, will remain in effect after termination.
